feat(nvm)!: remove soft-deprecated code 

BREAKING CHANGE: $NVM_LAZY, $NVM_LAZY_CMD, AND $NVM_AUTOLOAD cannot be
used anymore to tweak nvm plugin. Use zstyle-based configurations
instead.
